CYO Parents Code of Conduct

Medical Form
The athlete pre-participation form is to be completed and submitted to the Pastoral Designee before the athlete is eligible to participate in a practice or game.  The form contains the physical form, release form and emergency medical authorization information.  The pre-participation form is valid for 13 months from the date of the physical exam.  The form is available here
Eligibility request form

The Eligibility Request Form below is to be used for one individual athlete seeking permission to play on another member’s team.  The ERF must be approved by the CYO Office before an athlete is permitted to participate in any practices or games. Uniform Policy

The St. Joseph Parish Catholic Youth Organization Board of Directors has adopted a new policy for issuing uniforms as follows:

The St. Joseph Parish (SJP) Catholic Youth Organization (CYO) Board of Directors (Board) shall collect a "Uniform Security Deposit" of fifty dollars ($50), preferably by check, for each and every participant that is issued a SJP CYO uniform.  The Uniform Security Deposits shall be held by the Treasurer until such time that the SJP CYO Board instructs the Treasurer to deposit any or all of the Uniform Security Deposits into the SJP CYO bank account.

The Uniform Security Deposit shall be collected at the time the uniform is issued and/or distributed by the SJP CYO.

A date and time for a Uniform Distribution and/or Collection Event shall be established by the SJP CYO Board and published through SportsPilot and/or the SJP CYO Website.  The SJP CYO Board may use alternative means to publish such dates and times (i.e. Edline, Parish Bulletin, etc.).

The Uniform Security Deposit shall be refunded to each participant that returns their uniform, in good condition, during the Uniform Collection Event.

Any uniform that is not "Returned" by a participant within a fourteen (14) day Grace Period of the Uniform Collection Event shall be considered "Not Returned."  The SJP CYO Board shall act accordingly by directing the Treasurer to deposit the Uniform Security Deposits into the SJP CYO bank account for all uniforms deemed as "Not Returned."

A uniform that is "Returned," in good condition, after the fourteen (14) day Grace Period shall be subject to a fine of $25, hence the SJP CYO Board, at a timeframe established by the Treasurer, will refund one-half of the Uniform Security Deposit.

A uniform shall be considered "Returned" if it is received, in good condition, at the Uniform Collection Event or provided directly to the CYO Board Member responsible for uniforms (see SJP CYO Website).  Uniforms shall NOT be returned to the St. Joseph School, St. Joseph Parish Office, a Coach, or other Board Members, or they may be considered "Not Returned" and/or may be subject to a fine of $25.

At this time, SJP CYO High School Sports are not included as part of this policy.
